Frightening but not surprising: medical team cant find vein, execution of death row inmate halted in US

A death row inmate in the U.S. has been granted a reprieve. Tony Carruthers’ execution was postponed last Thursday in Tennessee, as the staff failed to locate a vein for the lethal injection, as reported by the Associated Press.

The Tennessee Department of Corrections stated that the medical staff had established a main line for administering the lethal drugs, but could not find a vein for a second injection site, which was necessary for the protocol. After an hour of attempts, the execution was canceled, according to a press release.

Following this, Tennessee Governor Bill Lee granted Tony Carruthers a one-year reprieve. Carruthers, who spent over thirty years on death row, has consistently maintained his innocence.

The 57-year-old was sentenced to death for the murders of Delois Anderson, her son Marcello Anderson, and Frederick Tucker in 1994.

Stacy Rector, the executive director of the Tennesseeans for Alternatives to the Death Penalty (TADP), described the failed execution attempt as “frightening” but not surprising. TADP has long raised concerns about serious problems related to lethal injections and called for greater transparency from the state.

In another case, Richard Knight, 47, was executed by lethal injection in Florida for the 2000 murders of a woman and her 4-year-old daughter. This execution marks the 14th carried out in the U.S. this year. Last year saw a record 47 executions, the highest since 2009 when 52 people were executed.

Most executions are performed by lethal injection, with 39 in 2025. Five were by nitrogen inhalation, a method introduced by Alabama in 2024, criticized by UN experts as a form of “torture,” and three by firing squad in South Carolina, the first in the U.S. since 2010.
